Objectives: The objective of tbe present study is to compare the success rates of two different periapical surgical techniques, the traditional technique with rotatory instruments and the ultrasonic technique, and evaluate the influence of other factors in the process of periapical lesions cure. Material and methods: 61 apicoectomies were carried out in 50 patients. In 23 patients we used the traditional technique and in 27 the ultrasonic technique. We made histological analysis of the periapical lesions. Radiographic and clinical assessment criteria were used one year befare periapical surgery. Results. Complete healing after 1 year was observed in 52% of the teeth treated through the use of the standard technique and 81% of those treated through the use of the ultrasonic technique. The size of the periapical lesion showed as a determinant factor although lesions treated with the ultrasonic technique had lower size. The histological findings did not affect the success rate. Conclusion: Ultrasonic technique in periapical surgery offers a better prognosis and increases the success average more than the use of rotatory instruments.
