Abstract
We present an upper lateral incisor with a periapical lesion of endodontic origin. Three years and six months after the root canal therapy showed apical repair and the presence of a new fistula comming from a lateral lesion. Intracanal endodontic therapy was iniciated. Internal anatomy showed one accesory palatal root. A complemental surgery shows endodontic success eleven years follow up.
