Nutcracker syndrome is an extrinsic compression of the left renal vein produced as a result of entrapment between the aorta and superior mesenteric artery by decreasing the normal angle between these two arterial trunks . Because of this phenomenon , hypertension of the left renal vein varices in the left gonadal vein and unilateral hematuria less frequently have chronic pelvic pain or dysuria is produced . It occurs equally in both sexes and its true prevalence is not known. It is a low incidence of disease and should be suspected in young patients , male or female, with longstanding symptoms without other conditions that warrant. Retrograde venography is the technique of choice for diagnosis. The treatment of nutcracker syndrome depends on the severity of the disease and the symptoms. It can be treated surgically or by endovascular procedures . We present the case of a patient in whom the diagnosis was delayed three years, but was finally treated by embolization of the left gonadal vein and stent placement in the left renal vein all the symptoms disappearing.
