Concentración del fragmento aminoterminal del pro-péptido natriurético cerebral en plasma como marcador biológico predictivo de mortalidad en las neumonías adquiridas en la comunidad

Objective: To evaluate the relationship between plasma concentration of the amino-terminal fragment of pro-brain natriuretic peptide (NT-proBNP) at the time of diagnosis of community-acquired pneumonia (CAP) in the emergency department and outcomes assessed as 30-day mortality. Methods: An observational prospective cohort study of patients with clinical and radiological diagnosis of CAP admitted consecutively to the emergency department from February 6, 2012 to February 24, 2013 was conducted. NT-proBNP was determined as the exposure factor, and the 30-day mortality rate was analyzed. Results: 338 patients were evaluated, and 287 (84,9%) were included in the study, 42,2 % women. The mean age was 66 years (SD 21). The mean values of blood NT-proBNP were 3.000 ng/l (SD 8.068) and median 492 ng/l (IQR 1.865). A total of 30 patients died within 30 days (10,45%). The mean values of NT-proBNP in patients who died were 14.035 ng/l (SD 19.271) compared with 1.711 ng/l (SD 3.835) in survivors (p<0,0001). Conclusions: NT-proBNP values at the time of diagnosis of CAP in the emergency department are a good predictor of early mortality within 30 days.
